MEDICAL FORMS
A medical form is required for your child to attend camp. Please complete and submit the medical form by TBD.
When you enter the page, you will be prompted to enter your name, your child's name, and your email address. Once that information is submitted, then you can begin the medical form.
You can save progress if you need to step away from the form and come back to it later.
CAMP GEAR
If you would like to order merchandise for your child or yourself, please follow the link to the online store. You must include the teacher and school in your order as this is how we will keep track of your order. Your child will receive their merchandise when they arrive at camp. We do not ship merchandise.
**Please note that this order must be completed at the same deadline as the medical form which is listed above. If it is placed after that date, we cannot ensure we will have the exact sizes due to supply chain issues.**
